Tesamorelin (TH9507) is a synthetic analogue of growth hormone-releasing hormone (GHRH 1-44), with a trans-3-hexenoic acid modification at the N-terminus that protects it from rapid degradation by plasma DPP-IV enzymes. Native GHRH has a half-life measured in minutes; tesamorelin's modification extends this significantly while maintaining the same receptor target and physiological GH release pattern. It's the most clinically studied GHRH analogue available.

GHRH Receptor vs Ghrelin Receptor

This is what separates tesamorelin from ipamorelin. Ipamorelin activates GHSR-1a — the ghrelin receptor — to trigger GH release. Tesamorelin activates GHRHR — the native GHRH receptor — to trigger GH release through the hypothalamic-pituitary axis's own signalling pathway. Both result in pituitary GH secretion, but through different receptor mechanisms. Combining them activates both pathways simultaneously, which is why they're frequently stacked in GH axis research protocols.

Visceral Adipose Tissue Research

Tesamorelin has been extensively studied for its effects on visceral fat accumulation — particularly in models where GH deficiency or suppression drives excess visceral adipose deposition.
